Preventing Hospital Errors

6000 "never events" happen every MONTH just among Medicare patients

Some 80% of medical malpractice lawsuits are grounded in preventable error -- the problem is negligent doctors, not greedy patients.  Doctors need to think about what they're doing every step of the way, not operate on autopilot.

Another small triumph

Those of you who've been around for a while know that I've been lobbying for years to make it illegal for anyone except a psych professional to place a psych diagnosis in a patient's medical records.  Too many of us with fibromyalgia and neurological diseases find ourselves being tagged with "depression" or "anxiety" or "work-o-phobia" by doctors who don't want to think a little harder to find a cause for the symptoms when "all blood tests are normal".
When I was at the doctor the other day, we were discussing the changes coming in ICD-10, and he mentioned that another change in the law is that if a medical professional puts a psych diagnosis on a form, they get penalized 25% on their charge for that appointment.
Hopefully, having it come out of their pockets will discourage such laziness, and they'll be forced to listen to the patient to get the correct answer instead of just leaping to the immediate conclusion that fatigue=depression.  (OK, wrote that down, I'm off the hook, a diagnosis has been made.  Who cares if it's right or wrong, I've fulfilled my obligation to attach a label to this patient.)
